What are the best child friendly hotels in Japan?

When I travel to Japan, there will be two adults and one child. I am wondering what accommodations they will make for my child such as by providing a cot or bed or if we should rent a second room beside the adult room. She is too young to stay by herself or at least I would not feel comfortable with that.
And, she will not just want to lie on the floor, so she would need her own twin sized bed or something comfortable. Currently she is as tall as me, so a tiny cot would not work.
Also, what hotels offer child friendly fun activities with other children on premises? I would be interested in finding hotels in various cities that offer the best child friendly accommodation, such as organized activities where children can mingle with others their own age. Please offer suggestions.
